Royal commissions and other formal inquiries are established when serious harm, institutional failure or matters of public importance demand closer examination.
They gather evidence, hear from experts and affected communities, identify failures and recommend reform. But publication of a report is not the end of the story.
Royal Commissions Examined asks what happens after the findings—when recommendations have been made, governments and institutions have responded, and responsibility shifts from investigation to implementation.
From Recommendations To Reform
Drawing on public records, historical material, expert analysis and lived experience, Episode Four considers how institutional learning is carried forward—and how meaningful reform can be recognised in practice.
It explores the distance that can emerge between what an inquiry recommends, what institutions promise and what people experience in the years that follow.
The episode does not begin with a predetermined conclusion. It asks how progress should be assessed, what accountability requires and whether lessons remain visible after public attention has moved elsewhere.
Completing The Shattered Series
Across the series, Shattered examines what happens after workplace injury: how systems developed, how care can become contested, and how financial pressure and workplace culture can shape recovery.
The final episode brings these questions together.
If harm has been identified and its consequences are understood, what responsibility follows? How do institutions demonstrate that lessons have been learned? And whose experience tells us whether reform is working?
